29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly. On 11/26/2024 and at times prior thereto, a Waldon drum handler attachment was available for use with a Hyster forklift truck to move 55-gallon drums. The capacity, operation and maintenance plates were not changed to reflect the capacity with this attachment. Use of attachments can affect operator visibility and forklift stability and capacity. This condition potentially exposed employee(s) to forklift hazards.

29 CFR 1910.22(c): The employer did not ensure that each employee used a safe means of access and egress to and from walking-working surfaces: Outside at the tool trailer: On November 26, 2024 and at times prior thereto, the ramp to access the tool trailer did not have guardrails on the sides. The maximum height of the ramp was approximately 50". This condition potentially exposed employee(s) to a height hazard which could result in falls.
