29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about November 19, 2024, at the worksite, 543 Bryant Avenue, Syracuse, NY 13204: The employer failed to ensure employees engaged in residential roof replacement were protected from 22 foot fall hazards by use of personal fall arrest systems, safety net systems, or guardrail systems.    b) On or about November 19, 2024, at the worksite, 543 Bryant Avenue, Syracuse, NY 13204: The employer failed to ensure employees engaged in residential roof replacement were protected from 12 foot fall hazards by use of personal fall arrest systems, safety net systems, or guardrail systems.    c) On or about November 19, 2024, at the worksite, 543 Bryant Avenue, Syracuse, NY 13204: The employer failed to ensure employees engaged in residential roof replacement were protected from 8 foot fall hazards by use of personal fall arrest systems, safety net systems, or guardrail systems.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about November 19, 2024, at the worksite, 543 Bryant Avenue, Syracuse, NY 13204:  The employer failed to ensure employees engaged in residential roof replacement, exposed to 22 foot fall hazards, were trained on the recognition of fall hazards and procedures to minimize those hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about November 19, 2024, at the worksite, 543 Bryant Avenue, Syracuse, NY 13204:  The employer failed to ensure a portable extension ladder an employee was using to access a roof had the side rails extended 3 feet above the access point.  The employee was exposed to a 12 foot fall hazard.
