29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On November 11, 2024, and at times prior thereto, keys were left in the ignition, in the "on" position, with the engine running, of an unattended Ditch Witch, model 255SX fitted with a drilling rod attachment, while servicing and maintenance was performed by an employee. NOTE: Abatement certification and documentation of corrective actions are required.
