29 CFR 1910.140(c)(12): Anchorages used to attach to personal fall protection equipment were not independent of any anchorage used to suspend employees or platforms on which employees work:. On or about November 19, 2024, an employee was working directly at the edge of a 3 story building where he was not adequate protected from falls such that this employee was connected to rolling platform (mule) that was being used to suspend a separate employee while they performing window washing activities. Additionally, this scaffold (rolling platform / mule) was not securely connected or anchored to the building in that there was excessive slack for the scaffold's safety lifeline.
