29 CFR 1910.28(b)(3)(i): 29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): On or about November 15, 2024 and times prior thereto, on the North-East side of warehouse roof, employees removing an internet antennae cable were not protected from falls when on the metal roof and on uncovered skylights at a height greater than four feet.

29 CFR 1910.30(a)(1):29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about November 18, 2024 and at times prior thereto, on the North-East side of warehouse roof, employee's access the roof without first being properly trained on the use of fall protection equipment.
