29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on shall be disconnected from all electric energy sources. Control circuit devices, such as push buttons, selector switches, and interlocks, may not be used as the sole means for deenergizing circuits or equipment. Interlocks for electric equipment may not be used as a substitute for lockout and tagging procedures. Bldg. 2404: On November 8th, 2024, and at times prior thereto, employee(s) were tasked to perform corrective maintenance work on an automatic transfer switch (ATS) unit whose energy source was not disconnected from circuits and/or equipment. Also, the automatic transfer switch (ATS) unit had not been locked and/or tagged out properly during servicing and maintenance activities. This condition exposed employee(s) to arc flash hazard(s).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed. Bldg. 2404: On November 8th, 2024, and at times prior thereto, employee(s) were tasked to perform corrective maintenance work on specific work order(s) for the day such as; electrical, plumbing, Heating, Ventilation, and Air Conditioning (HVAC) and painting. An employee was hospitalized due to severe charring, blistering, and damage to both front sides of the arms, hands and face from an Arc-Flash, due to, not using appropriate electrical protective equipment.

Bldg. 2404: On November 8th, 2024, and at times prior thereto, employee(s) were tasked to perform corrective maintenance work on specific work order(s) for the day such as electrical, plumbing, Heating, Ventilation, and Air Conditioning (HVAC) and painting. An employee was hospitalized due to severe charring, blistering, and damage to both front sides of the arms, hands and face from an Arc-Flash that occurred from the Automatic Transfer Switch (ATS) Unit. A late notification to OSHA was made on November 14, 2024.
