29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Location:  Building 3 P3 Cleaning Line; Ruesch # 21 slitter; Aida Stamping Press  Instance A:  Written LOTO procedures were not developed for the P3 Cleaning Line or the Aida Stamping Press Instance B:  The written LOTO procedure for the Ruesch # 21 slitter does not address mechanical or pneumatic hazards. Instance C:  Production employees are not trained  to, and do not, lock out machinery prior to performing servicing and maintenance tasks. Instance D:  The company is not performing periodic inspections of its LOTO procedures.
