29 CFR 1910.136(a):. The employer did not ensure that each affected employee used protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures. At 4946 Nash Road, Scott City, MO 63701, on November 19, 2024 and at times prior thereto, employees were exposed to hazards associated with piercing the sole of the shoe and foot crushing injuries from pallets falling on feet during the construction process and loose nails on the floor piercing the soles of shoes.

29 CFR 1910.178(m)(4): The employer did not prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck. At 4946 Nash Road, Scott City, MO 63701, on November 19, 2024 and at times prior thereto, were exposed to caught between hazards associated with a self-dumping dumpster when dumped. Employees were able to place their hands in proximity of the forklift blades and load causing a pinch point.
