29 CFR  1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  In the service department, employees engaged in grinding, and sharping activities using the pedestal grinder were exposed to caught-in hazards in that the work rest opening were not maintained at a maximum of one-eight inch. The opening from the abrasive wheel to the work rest was found to be one-quarter inch.

29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  In the service department, employees engaged in grinding, and sharping activities using the pedestal grinder were exposed to struck by hazards in that the adjustable tongue guard opening was not maintained at a maximum of one fourth inch. The opening from the abrasive wheel to the tongue guard was found to be one and three-sixteenth inch.

29 CFR  1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  In the service department, employees engaged in maintenance activities were exposed to chemical hazards in that a written hazard communication program had not been developed, implemented and maintained at the workplace. Employees were handling chemicals including but not limited to engine oil, hydraulic fluid, and gasoline . The hazards associated with these chemicals include: skin and eye irritation, drowsiness and/or dizziness, throat, lung and respiratory tract irritation, silicosis/siderosis, vomiting and/or diarrhea, suspected of causing cancer and may be fatal if swallowed and enters airways.

29 CFR  1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use  In the service department, employees engaged in maintenance activities were exposed to chemical hazards in that they were not provided access to the Safety Data Sheets (SDSs) for chemicals including but not limited to engine oil, hydraulic fluid, and gasoline. The chemical hazards associated with these chemical include: skin and eye irritation, drowsiness and/or dizziness, throat, lung and respiratory tract irritation, silicosis/siderosis, vomiting and/or diarrhea, suspected of causing cancer and may be fatal if swallowed and enters airways. Additionally, gasoline can act as a fueling agents due to its extreme flammability as a liquid and vapor.

The chemical hazards associated with gasoline include: skin and eye irritation, drowsiness and/or dizziness, throat, lung and respiratory tract irritation, silicosis/siderosis, vomiting and/or diarrhea, suspected of causing cancer and may be fatal if swallowed and enters airways. 29 CFR  1910.305(g)(2)(ii): No. 14 and larger cord was not repaired so that the repair retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced:  In the service department, employees engaged in maintenance activities were exposed to electrical shock and fire hazards due to a damaged and improperly repaired extension cord. Electrical tape was used to protect the spliced wires.
