29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: 40 Front Street, Belchertown, MA 01007: On or about November 20, 2024, and at times prior, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed, and the signature of the person who conducted the training for the employees who were performing metal roofing and exposed to fall hazards of approximately 12 feet above a lower level.
