29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface:    On or about November 20, 2024, an employee working from the raised platform attached to the 48-inch mill located in Building B was exposed to falling onto the mill because two sections of the top rail located on the mill side were approximately 36 inches high.

Mill 1 did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled.  b. 48" Mill did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled.  c. Stolarczyk Re-Coiler did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled.  d. Schlebach CTL did not include steps for testing and verifying that all electrical energy sources had been dissipated and effectively controlled.  e. Side Shear did not include steps for testing and verifying that all electrical and pneumatic energy sources had been dissipated and effectively controlled.  f. Back Shear did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled.  g. Mill 5 did not include steps for testing and verifying that all electrical and hydraulic energy sources had been dissipated and effectively controlled.  h. The Jorns Normaline folder lockout procedures did not include step for testing and verifying that electrical and hydraulic energy sources had been dissipated and effectively controlled.  i. The Thalman folder lockout procedures did not include step for testing and verifying that electrical and hydraulic energy sources had been dissipated and effectively controlled.  j. FJ-10 Mill lockout procedures did not include step for testing and verifying that electrical and pneumatic energy sources had been dissipated and effectively controlled.
