29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Clean Room D: On or about November 15, 2024, an employee suffered a finger fracture while inserting her fingers into the point of operation on the PurePouch LLC packaging machine (Serial # PP107-020; Model: PP107A) . The employee was exposed to an amputation hazard. The employer did not provide proper point of operation guarding on the machine. Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).
