29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) Production and packaging area/ Hop Doser & Brewhouse - On or about 11/20/2024, and at times prior, employees were exposed to struck-by hazards in that, the employer failed to ensure that each power industrial truck (PIT) operator was evaluated for safe operation while operating Doosan electrical forklifts.
