29 CFR 1926.502(h)(1):The employer shall designate a competent person to monitor the safety of other employees and the employer shall ensure that the safety monitor complies with the following requirements: a) On or about November 18, 2024, the employer failed to fully implement their chosen fall protection system-comprising a warning line system and safety monitoring system-for employees working on a low-sloped roof while installing roofing material. Specifically, the employer did not designate a competent person to monitor the safety of the employees. As a result, employees were exposed to fall hazards of approximately 33 feet and 5 inches.

29 CFR 1926.503(a)(2)(vii): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: The role of employees in fall protection plans; a) On or about November 18, 2024, the employer failed to ensure that employees conducting roofing work on a low-sloped roof with a 2:12 pitch were trained and knowledgeable about their roles in the employer's fall protection plan. Specifically, no employee was designated or trained to perform safety monitor duties, resulting in employees being exposed to a fall hazard of 33 feet, 9 inches.
