29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  On or about November 19, 2024, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about November 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about November 19, 2024, at the above addressed jobsite, an employee performing residential roofing operations, eight (8) feet ten (10) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about November 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, seven (7) feet above a lower level, were not protected from falls, thereby exposing the employees to fall hazards.  Rivera's Remodeling Exteriors Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1605159, citation number 1, item number 1, and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 20656 Michigan St. in Crest Hill, Illinois.

On or about November 19, 2024, at the above addressed jobsite, employees had been provided fall protection training; however, the employer failed to maintain a written certification record of the training.  Rivera's Remodeling Exteriors Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1605159, citation number 2, item number 1, and was affirmed as a final order on December 7, 2022, with respect to a workplace located at 20656 Michigan St. in Crest Hill, Illinois.
