Employee(s) were working on the roof of a residential structure. The employees were installing shingles while working at a height of approximately 11 feet 6 inches measured from the ground to eave without fall protection.

This was most recently documented on or about November 21, 2024 at a construction jobsite located at or about 1426 West Silverado Street, Lincoln, Nebraska. Employee(s) were shingling a roof where a ladder was used by the employees that was not extended over the upper landing surface.
