29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:   Worksite located 7610 E. 85th Terrace, Kansas City, Missouri- Employees were exposed to overhead hazards while  working and walking under employees, materials and equipment and were not protected from impact by the use of hard hats.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Worksite located at 7610 E. 85th Terrace, Kansas City, Missouri- Employees were exposed to fall hazards while tearing off the siding  on the third level deck and was not protected from falling by the use of any means of fall protection.

29 CFR  1926.1053(a)(8): A metal spreader of locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder is being used.    Worksite located at 7610 E. 85th Terrace, Kansas City, Missouri- Employee was exposed to a fall hazard in that the employee was using a step ladder on top of the 3rd level in which the metal spreader was not engaged and was leaning on the wall where it could slide down the wall.
