On or about November 19, 2024, at the above addressed jobsite, an employee using a handheld battery powered circular saw was not wearing eye protection, thereby exposing the employee to hazards associated with eye injuries.

On or about November 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet five (5) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards. b. On or about November 19, 2024, at the above addressed jobsite, an employee performing residential roofing operations, nine (9) feet four (4) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards. c. On or about November 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet two (2) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.
