29 CFR 1915.116(b):Loads were not safely rigged before being moved: (a) on or about 11-27-2024 and at times prior thereto, Shop 51 workers, working in building 427 on the first floor, using a CAT 3 wall mount jib crane, serial number AH-142, were exposed to struck by, entanglement, and crushing hazards, in that a 550 pound load was lifted out of a crate without being securely attached to the lifting apparatus.

29 CFR 1915.116(q):An employee was permitted to place him/herself in a hazardous position between a swinging load and a fixed object. (a) on or about 11-27-2024 and at times prior thereto, Shop 51 workers, working in building 427 on the first floor, using a CAT 3 wall mount jib crane, serial number AH-142, were exposed to struck by, entanglement, and crushing hazards, in that rules were not implemented or enforced to prevent the workers from becoming caught between the suspended load and the crate it was being removed from.
