29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: (a) 3147 Century St, Colorado Springs, CO 80907: On or about November 19, 2024, and at times prior, the employer did not ensure that respirators were cleaned and disinfected using the procedures required by this standard. Employees used ethanol containing heptane to clean respirator facepieces, chemicals which may result in skin irritation, dermatitis, and may affect the central nervous system via inhalation.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes were not provided within the work area for immediate emergency use: (a) 3147 Century St, Colorado Springs, CO 80907: On or about November 19, 2024, and at times prior, the employer did not ensure that an eyewash station was available where employees were exposed to a corrosive chemical. Employees measure and dispense Bacti-Free Third Sink Sanitizer, a corrosive liquid, in the gummy room for cleaning throughout the facility. Eye exposure to the chemicals in use in the employer's workplace may result in serious eye damage including loss of vision.
