29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, 3170 William Pitt Way, near Gate D, Pittsburgh, PA: On or about November 21, 2024, during water/fire system line work, the employer did not provide appropriate cave in protection for an employee in an excavation that was determined to be at least approximately 6 1/2 feet in depth.
