29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces. a) On or about November 19, 2024 at complex 46: employees were exposed to trip hazards during access and egress of the fixed ladder, in that the turnbuckle padeye was set approximately 7 inches from the base of the ladder.
