29 CFR 1910.134(d)(1)(iii) as required by 1926.103: When the employer was unable to identify or reasonably estimate the employee exposure, the employer did not consider the atmosphere to be IDLH:    a. Windham Town Hall Basement - On or about November 12, 2024, employees working near a Whiteman Multiquip gasoline powered concrete trowel, Model J36H90H, Serial # YI0167080 were exposed to carbon monoxide from the gasoline engine exhaust.  The employer did not reasonably estimate employee exposure to carbon monoxide and did not consider the atmosphere to be immediately in danger of life or health (IDLH).      Note:  Acute exposure to high levels of carbon monoxide may result in symptoms of central nervous system toxicity including, but not limited to headache, dizziness, drowsiness, weakness, nausea, vomiting, confusion, disorientation, irritability, visual disturbances, convulsions, coma, and death.  Following acute-onset effects, delayed development of neuropsychiatric impairment may occur within 1 to 4 weeks of exposure, with symptoms including inappropriate euphoria, impaired judgment, poor concentration, memory loss, cognitive and personality changes, and psychosis.

29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a. Windham Town Hall Basement - On or about November 12, 2024, employees working near a Whiteman Multiquip gasoline powered concrete trowel, Model J36H90H, Serial # YI0167080 were exposed to carbon monoxide from the gasoline engine exhaust.  The employer did not instruct employees to avoid the indoor use of equipment powered by internal combustion engines.      Note:  Acute exposure to high levels of carbon monoxide may result in symptoms of central nervous system toxicity including, but not limited to headache, dizziness, drowsiness, weakness, nausea, vomiting, confusion, disorientation, irritability, visual disturbances, convulsions, coma, and death.
