(b) Assembly Area - On or about November 19, 2024, the employer did not provide guarding over all unused portions of the saw blade on equipment such as the Craftsman Vertical Bandsaw.  No abatement certification or documentation is required for this item.

(a) Assembly Area - On or about November 21, 2024, the employer did not ensure that the right wheel of the Dayton Grinder, M/N: 1Z853, was equipped with a work rest.  (b) Repair Area - On or about November 21, 2024, the employer did not ensure that the left grinding wheel of the Baldor Grinder, M/N: 612, S/N: B131121003, was equipped with a work rest.  (c) Repair Area - On or about November 21, 2024, the employer did not ensure that the work rest on the right grinding wheel of the Baldor Grinder, M/N: 612, S/N: B131121003, was adjusted to a maximum opening of one-eighth of an inch to support offhand grinding work.  No abatement certification or documentation is required for this item.

(b) Repair Area - On or about November 21, 2025, the employer did not ensure that all abrasive wheel machinery was equipped with a tongue guard, such as on the left abrasive wheel of the Baldor Grinder, M/N: 612, S/N: B131121003. 29 CFR  1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:   The employer, Scott Leasing Company, Inc., did not enter each recordable injury or illness on the OSHA Form 300, Log of Work-Related Injury and Illness or complete OSHA Form 301, Injury and Illness Incident Report within seven calendar days of receiving information that a recordable injury or illness had occurred. Recordable injuries and illnesses included the following instance:  (a) On or about November 21, 2025, the employer had not entered a recordable injury on the OSHA Form 300 and OSHA Form 301 within seven calendar days of receiving information that a recordable injury or illness had occurred on October 18, 2024, when a worker sustained a laceration to the left hand, which resulted in several days of restricted work duties.   No abatement certification or documentation is required for this item.
