29 CFR 1910.67(c)(2)(iv): The employer did not ensure that employees always stand firmly on the floor of the basket, and did not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position: (a) On and before November 22, 2024, two employees installing holiday lights were not standing firmly on the floor of the basket of the Genie S-60 J Telescopic Boom Lift as required. Employees were exposed to falls from heights of approximately 20 feet.

29 CFR 1910.67(c)(2)(v): The employer did not ensure that a personal fall arrest system or travel restraint system that meets the requirements in Subpart I of this part was worn and attached to the boom or basket when working from an aerial lift: (a) On or before November 22, 2024, employer did not ensure that employees were protected from injury, in that two employees installing holiday lights using a Genie S-60 J Telescopic Boom Lift, and were not utilizing a personal fall arrest system. Employees were exposed to falls from heights approximately 20 feet.
