29 CFR  1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  On or about November 22, 2024, and at times prior thereto, employees were exposed to hazards of visible mold in the following locations:  a.  first floor laundry room, two air conditioner ceiling vents; b.  first floor office behind front desk, storage closet door trim/facing; c.  first floor hallway across from Room 111/the laundry room, cove base and carpet; d.  Room 325, air conditioner louvers/vents; e.  third floor storage room adjacent Room 325, air conditioner ceiling vent; f.  second floor storage room across from the south side stairs, air conditioner ceiling vent; g. second floor electrical room adjacent Room 225, back right corner directly across from the entrance door, ceiling surface adjacent the ceiling hole; h.  Room 219, air conditioner louvers/vents; i.  Room 217, air conditioner louvers/vents and below the bathroom mirror; and, j.  Room 214, air conditioner louvers/vents and bathroom shower caulking.

29 CFR  1910.303(g)(1)(ii):  The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  On or about November 22, 2024, and at times prior thereto, three electrical rooms were had electrical panels blocked by materials in the following instances:  a. first-floor electrical/television/video room; b. second-floor electrical room; and, c. third-floor electrical room.

29 CFR  1910.1200(e)(1):  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about November 22, 2024, and at times prior thereto, employees engaged in housekeeping, laundry, and maintenance activities were exposed to hazardous chemicals.  The employer had not developed a program which would address labeling and other forms of warning on chemical containers, Safety Data Sheets, and employee information and training on the hazards associated with chemicals utilized at this site.      The written program must also contain the following:     a.  List of all hazardous chemicals on site,     b.  Methods the employer will use to inform employees of the hazards of non-routine tasks involving chemicals, such as a spill,     c.  Hazards of chemicals contained in piping that is not labeled, and     d.  Method the employer will use to inform other employers (contractors) of the chemicals their employees might be exposed to while performing duties at this site.
