29 CFR 1910.212(a)(2): Guards were not affixed to the machine where possible or secured elsewhere when attachment to the machine was not possible. On or about November 19, 2024, employees performing inspection activities on the Line-5 Valve Hole Robot, Machine 3473, were exposed to struck-by hazards while attempting to unjam the pneumatic mesh panel guard and the stationary portion was not fully affixed.
