29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a) 14571 N Navajo St. Westminster, CO 80023: On or about November 23, 2025, and at times prior, employees were exposed to fall hazards of approximately 20 feet while conducting roofing activities on the top plate of the second level without utilizing personal fall protection or guardrails.  The employer did not provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.  Employees worked on the top plate of the structure without personal fall protection or guardrails.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 14571 N Navajo St. Westminster, CO 80023: On or about November 23, 2025, and at times prior, employees were exposed to fall hazards while using an extension ladder that was not extended at least three feet above the landing surface being accessed.
