29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about November 25, 2024, employees performed set-up work tasks on BOE machine model number - 615, Serial number - 4638, without using developed machine specific hazardous energy control procedures. This exposed employees to amputation and caught-in hazards in the event of inadvertent start-up of the BOE machine. (b) On or about November 25, 2024, employees performed set-up work tasks on BOE machine, model number - 601, Serial number - 4637, without using developed machine specific hazardous energy control procedures. This exposed employees to amputation and caught-in hazards in the event of inadvertent start-up of the BOE machine.

29 CFR 1910.147(d): The application of energy control (lockout or tagout) actions were not done in the following sequence: (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: (a) On or about November 25, 2024, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of hazardous energy sources were performed prior to employees setting up the Booklet Open End machines (BOE's). Employees performed set-up work tasks on BOE machine, model number - 601, Serial number - 4637, without deenergizing each source of energy and placing a locking device on the energy isolating device for each identified energy source within the written procedure. The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.

(a) On or about November 25, 2024, the employer did not ensure that each BOE's operators and adjustors were adequately trained on the methods and means necessary for the isolation and control of all hazardous energy sources prior to starting set up work. Adjustor and operator employees were not trained to and authorized level, exposing the employees to amputation, caught-in and entanglement hazards.

On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the energized converting and envelope machines for the BOE model number - 601, serial number - 4637: The guard for the converting section was in an up/open position during operation exposing employees to amputation and entanglement hazards from rotating parts. c. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation and entanglement hazards when working at and around the energized converting and envelope machines for the BOE model number - 620, serial number - 5504: The guard for the converting section was in an up/open position during operation exposing employees to amputation and entanglement hazards from rotating parts. d. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation hazard when working at and around the Auto String & Button 837 machine, serial number - 4096: The machine was not guarded, creating an opening for employees to reach into the point of operation, exposing employees to pinch point and amputation hazard. p. On or about November 25, 2024, machine guarding was not properly installed, bolted, and, or fastened in place to protect employees from amputation hazard when working at and around the clasp machine 701, serial number - 513: The machine was not guarded, creating an opening for employees to reach into the point of operation, exposing employees to pinch point and amputation hazard.
