OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. In the Main Production Area, located at 6940 Stuart Avenue, Jacksonville, FL: On or about November 26, 2024, the procedures and reference pictorials in the Z-Flashing and Changeover Process Procedure Document #C12-WI-0035 provided machine operators with arguably confusing directions for machine guard positioning and operator hand placement, exposing employees to a laceration hazard. Among other methods, feasible and acceptable means of abatement would be to: 1. Revise Document # C12-WI-0035 with photos to depict correct positioning of the machine guards and operator hands for each step of the setup and changeover procedure for the Alvarez Roll form Z- Flash Machine. 2. Retrain operators on the correct guard positioning and operator hand locations for each step of the setup and changeover procedure."
