29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: (a) On and before November 11, 2024, the controlling employer did not ensure a safety program was developed and implemented for the work environment, to include regular inspections made by a competent person of the jobsite, materials, and equipment, in that approximately 22 subcontractor employees were working next to roof edges on three story multi-family apartment buildings without the use of a fall protection system. The subcontractor employees were removing flashing and shingles on roofs with a pitch of 6-in-12. This condition exposed subcontractor employees to fall hazards of approximately 40 feet.
