29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Location: Worksite On or about November 27, 2024 employees were exposed to fall hazards of over 20 feet while working on a pump jack scaffold to perform roofing activities, where the employer did not ensure that frequent and regular inspections of the site were made by a competent person.

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. Location: Worksite On or about November 27, 2024, employees were exposed to fall hazards of over 20 feet while performing roofing activities while working from unguarded pump jack scaffolding, where the employer did not ensure that fall protection was in place.
