On or about November 27, 2024, at the above addressed jobsite, employees working on the ground, under a worker performing residential roofing operations, were not wearing a hard hat to protect against falling materials and debris.  Fernandez Renovation Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1763057, citation number 1, item number 1 and was affirmed as a final order on September 16, 2024, with respect to a workplace located at 8760 W Richard St. River Grove, IL 60171.

On or about November 27, 2024, at the above addressed jobsite, employees performing residential roofing operations, seventeen (17') feet nine (4") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about November 27, 2024, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet three (3) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Fernandez Renovation Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1763057, citation number 1, item number 2 and was affirmed as a final order on September 16, 2024, with respect to a workplace located at 8760 W Richard St. River Grove, IL 60171.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a. On or about November 27, 2024, the portable ladders used to access the roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.  b. On or about November 27, 2024, the portable ladders used to access the second story roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.  Fernandez Renovation Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1763057, citation number 1, item number 3 and was affirmed as a final order on September 16, 2024, with respect to a workplace located at 8760 W Richard St. River Grove, IL 60171.
