29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to possible injuries: (a) MPA warehouse, vicinity of dock 10: On or about November 27th, 2024, and at times prior, the employer exposed employees to fire related hazards, in that access to fire extinguisher was blocked by trash and equipment. (b) MPA warehouse, at location S8 and T5, West side of facility: On or about November 27th, 2024, and at times prior, the employer exposed employees to fire related hazards, in that access to fire extinguishers was blocked by trash and equipment. (c) MPA warehouse, at location N4 and N5, center of facility: On or about November 27th, and at times prior, the employer exposed employees to fire related hazards, in that access to fire extinguishers was blocked by trash and equipment.

29 CFR 1910.176(a): Aisles and passageways were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard. (a) Memphis MPA, exit/entrance to PIT storage and charging area: On or about November 27, 2024, and at times prior, the employer exposed employees to struck by hazards, in that PIT lanes were blocked by OTRs being used to load ULD containers, forcing Power Industrial Truck (PIT) operators to drive without being able to see clearly through a congested area where pedestrian employees were moving packages from the OTRs to the ULDs. (b) Memphis MPA, throughout the facility: On or about November 27, 2024, and at times prior, the employer exposed employees to struck by and collision hazards, in that PIT lanes being used for two-way PIT traffic and pedestrian traffic, were blocked by OTRs, APCs, and dunnage, forcing PIT operators to pass each other in close proximity while weaving between the containers.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) USPS Memphis MPA, warehouse floor: On or about November 27, 2024, and at times prior, employees operating power industrial trucks and employees working in the warehouse were exposed to struck by hazards. Employee(s) operating power industrial trucks had not been re- certified every three years. The USPS Memphis MPA was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1698831, citation number 1, item number 1 and was affirmed as a final order on January 31, 2024, with respect to the workplace located at 4550 Swinnea Road, Memphis, TN.
