29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a) On or about November 11, 2024, the employer failed to ensure that employees utilized a proper fall protection system while performing roofing work on a residential roof with an 8:12 pitch. As a result, employees were exposed to fall hazards from a height of approximately 16 feet 9.5 inches

29 CFR  1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.   a.) On or about November 11, 2024, the employer failed to provide a written certification record of fall protection training for employees who were exposed to fall hazards while conducting roofing work on an 8-12 pitch residential roof. Employees were exposed to a fall height of 16' 9.5".

29 CFR  1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a.) On or about November 11, 2024, the employer failed to report an employee's in-patient hospitalization within the required 24-hour period following a workplace incident.
