29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Establishment, Warehouse area - On or about November 27, 2024, employees operate Crown SP4050 order pickers with large rubber bands, waste bags, plastic trays, and reach poles attached to the side gates. The arms/side gates are the safety interlock for the machine. Employees were not able to raise or lower the side gates to enter or exit the machine.
