29 CFR 1910.106(e)(6): In locations where, flammable vapors may have been present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition such as but not limited to open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about November 27, 2024, and at times prior thereto, employees conducted oil and natural gas separation equipment maintenance and servicing operations while flammable vapors were present without adequate precautions to eliminate all sources of ignitions.
