29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about December 03, 2024, employees were exposed to a fall hazard of 12.3 feet above concrete while completing work on a copper dome from a low slope commercial roof located at 4548 Wallings Road, NORTH ROYALTON, OH 44133. The employer did not ensure the extension ladder used to access the roof extended at least 03 feet above the upper landing surface. Rustic Slate Roofing, LLC, was previously cited for a violation of this occupational safety and health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1626763, citation number 1 item number 2 and was affirmed as a final order on 11/14/2022, with respect to a workplace located at 2909 Drummond Road, CLEVELAND, OH 44120.
