29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. At the shop area, located at 2361 Dennis Street, in Jacksonville, Florida: On or about December 3, 2024, the employer exposed employees to amputation hazards, in that the employer had not established an effective energy control program, consisting in energy control procedure, employee training and periodic inspections to ensure that unexpected energizing, startup or release of stored energy were isolated before allowing employees to perform servicing and/or maintenance on a machine or equipment such as but not limited to the metal table cutting saw.

At the storage area of the facility, located at 2361 Dennis Street, in Jacksonville, Florida: On or about December 3, 2024, the employer exposed employees to struck-by and crushed-by hazards, in that the employer did not ensure that each Powered Industrial Truck (forklift) operator had been trained and evaluated as required by this paragraph (l), prior to the employees operation of the truck.
