29 CFR 1910.178(n)(1): Traffic regulations including authorized plant speed limits were not observed: Warehouse Dock: On or about October 19, 2024, the employer did not ensure that all plant speed limits were observed while employees operate powered industrial trucks including, but not limited to, Raymond Pallet Center Riders. Employees were exposed to struck by injuries in the warehouse when a pallet operator traveled at high speeds before hitting stacked pallets that struck a pedestrian nearby.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: Warehouse Dock: On or about October 19, 2024, the employer did not ensure that industrial truck drivers were required to look in the direction of and keep a clear view of the path of travel while operating powered industrial trucks that included, but were not limited to, Pallet Riders, which were operated in the warehouse. Employees were exposed to struck by injuries when a pallet operator did not have a clear view of the path of travel before crashing into pallets that injured another employee.
