29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1 and Citation 2 Item 1.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On or about December 7, 2024, five employees transferring shingles on the roof of an apartment complex were observed working at heights greater than 6 feet without being  protected by guardrail systems, safety net system, or personal fall arrest system, nor were they provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  1) Davilas Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1743712, citation number 2, item number 2 and was affirmed as a final order on 11/21/2024, with respect to a workplace located at 320 E Conant St Portage, WI 53901.  2) Davilas Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1715082, citation number 2, item number 1 and was affirmed as a final order on 3/26/2024, with respect to a workplace located at Lot Number 6672 Honey Comb Lane Deforest, WI 53532  3) Davilas Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1710327, citation number 2, item number 1 and was affirmed as a final order on 1/16/2024, with respect to a workplace located at 5 Homewood Cir Madison, WI 53704. 4) Davilas Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1702342, citation number 2, item number 1 and was affirmed as a final order on 1/16/2024, with respect to a workplace located at 6649 Gehrig Place De Forest, WI 53532. 5) Davilas Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1596274, citation number 1, item number 1and was affirmed as a final order on 9/14/2022, with respect to a workplace located at 154 Pheasant Run Johnson Creek, WI 53038.
