On or about December 4, 2024, at the job site, 3000 Fiske Blvd, Rockledge, FL, employees were working inside a 50-foot-long trench and did not have any safe means of egress/ingress to the 6-foot-deep trench.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a). On or about December 4, 2024, at the jobsite, 3000 Fiske Blvd, Rockledge, FL, employees working inside a 6-foot deep trench, were not protected from excavated (spoils) materials falling back into the trench by placing those materials 2 feet from the trench. The spoils pile was excavated and placed at the trench opening and not set back 2 or more feet from the edge.

On or about December 4, 2024, at the jobsite, 3000 Fiske Blvd, Rockledge, FL, employees were exposed to cave-in hazards while working without a protective system inside an approximate 6-foot deep, 50-foot-long, 6-foot-wide trench with near vertical walls.
