On or about December 4, 2024, at the job site, 3000 Fiske Blvd, Rockledge, FL, employees were working inside a 50-foot-long trench and did not have any safe means of egress/ingress to the 6-foot-deep trench.

On or about December 4, 2024, at the jobsite, 3000 Fiske Blvd, Rockledge, FL, employees working inside a 6-foot deep trench, were not protected from excavated (spoils) materials falling back into the trench by placing those materials 2 feet from the trench. The spoils pile was excavated and placed at the trench opening and not set back 2 or more feet from the edge.
