On or about December 4, 2024, employees on the west side of the site were in an excavation 4 feet or more in-depth that did not have a stairway, ladder, or other safe means of egress.

The competent person did not inspect the excavations, the adjacent areas, and protective systems before the start of work and as needed throughout the shift. On or about December 4, 2024, employees performed utility work on the west side of the site in an excavation five feet or more in-depth that a competent person did not inspect. On the site's west side, employees performed utility line work in an excavation that was not inspected throughout the shift for evidence of a situation that could have resulted in possible cave-ins. The on-site supervisor would have discovered that the excavation was deeper than four feet, and a ladder or ramp would have been needed for egress. If the excavation had been inspected per the standard, the competent person would have discovered no cave-in protection in the excavation that exceeded five (5) feet in depth. A competent person would have determined that a means of egress deeper than four (4) feet was not present in the excavation.
