29 CFR  1910.133(a)(2):The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  On or about December 30, 2024, employees who operated forklifts within truck trailers to unload parts and/or accessed the robot weld area to deliver parts were exposed to eye hazards from flying particles and weld slag in that prescription eye lenses did not incorporate side protection which complied with ANSI Z87.1     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields:  On or about December 30, 2024, the employer did not ensure that forklift operators were protected from eye injury.  Prescription lenses did not incorporate eye protection to protect against flying particles when unloading truck trailers or welding slag when accessing the robot weld area(s) to deliver parts.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
