29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width, by the use of a safety monitor alone: a) Job site, at/near 4552 McKnight Road, Pittsburgh, PA: On or about December 4, 2024, during roofing work, the employer did not ensure that an employee, working outside of the flag line, used appropriate fall protection systems/devices where fall hazards of up to approximately 15 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, at/near 4552 McKnight Road, Pittsburgh, PA: On or about December 4, 2024, during roofing work, the employer did not ensure that the ladder used to get on/off the roof was extended at least 3 feet above the roof line in that it was only extended to an area below the edge of the roof, exposing an employee to a fall hazard of approximately 15 feet.
