1910.30(c)(3) 29 CFR 1910.30(c)(3): The employer did not retrain an employee when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicated that the employee no longer had the requisite understanding or skill necessary to use equipment or perform the job safely. 29 CFR 1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): (a) 230 North College Street, Palmyra, PA; Smokehouse - On or about November 6, 2025, employee(s) were exposed to fall hazards of at least approximately 7 feet to the lower level while climbing and working from the boards used to hang meat for smoking. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) 230 North College Street, Palmyra, PA; Production room to loading dock - On or about November 6, 2024, employee access to the exit path from the emergency exit was blocked at the exit discharge by a tractor trailer which was parked at the loading dock. Employees were exposed to evacuation hazard(s). Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
