29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about December 5, 2024, the employer failed to ensure the data plates on the powered industrial trucks using 25 foot long probe attachments were updated to show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about December 5, 2024, the employer failed to ensure the unused portion of the rotating band saw blade on the Tannewitz Band Saw located in the Blow Mold Room was not reduced to within 1/4 inches of the stock while making 3 cuts within inches of the employee's hands.
