29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) On or about December 5, 2024, at the Die Shop: employees were exposed to amputation hazards while clearing a jam in the #4 cavity of the 75.5mm die of the Medesa-Webmaster RGD 500 Machine (M4068.0913, M4068), in that, a Lockout/ Tagout device was not affixed.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. a) On or about December 5, 2024, at the Die Shop: employees were exposed to embolism hazards, in that the employer allowed the use of compressed air between 80 and 100 PSI to clean surfaces and product parts.
