29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Health Department Roof: On or about December 4, 2024 and times prior, ten employees were installing shingles on the steep-sloped roof of a one-story commercial building. The employees were not using a personal fall arrest system or a fall protection system exposing them to fall of up to approximately 11 feet 7 inches to ground. b) Detached Storage/Garage Roof: On or about December 4, 2024 and times prior, an employee was loading and standing on stacks of shingles on the steep-sloped roof of a detached storage/garage building. The employee was not using a personal fall arrest system or a fall protection system exposing them to fall up to approximately 11 feet 7 inches to ground.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Detached Storage/Garage Roof: On or about December 4, 2024, an employee was loading stacks of shingles on the steep-sloped roof of a detached storage/garage building, approximately 11 feet 7 inches above grade. The extension ladder being used to access the upper landing surface extended approximately 1 1/2 feet above the edge of the roof.
