29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about December 4th, 2024, employees engaged in maintenance related activities located at 1112 Madison Street, Fort Calhoun, Nebraska 68023, were exposed to in running nip point and rotating parts hazards in that an employee was attempting to clear a jam on the running Trim 7 Press Grinder without isolating the energy sources for the machine. He got his hand caught in the grinder, amputating multiple fingers on his right hand.

On or about December 4th, 2024, employees engaged in maintenance related activities located at 1112 Madison Street, Fort Calhoun, Nebraska 68023, were exposed to electrocution and fire hazards in that two ends of 480V/30 AMP cable that was ran from a junction box to a motor located under trim press #7 was damaged and was being used during normal production.

The employer was using the cables during normal production.

The cord in question were not being inspected for defects on the cable.
